Pain, the Lady of Whips, is the blind matron of affliction, the cruel saint of revelation, and the red-veiled goddess of transfiguring suffering. Myths claim she was born of violence itself—a maiden of silence and shadow impregnated by either a sea-giant’s scream or the howling breath of winter. Her womb birthed pain in all its forms: hunger, sickness, heartbreak, and the ache of longing. These were not curses. They were gifts.
Her worshippers teach that every soul is a swollen, septic wound at birth. Pain is the scalpel, and agony the cure. Suffering unmasks the lies we wear to survive comfort—it strips the soul bare and leaves it open to the divine. Through discipline, self-denial, and endured torment, one becomes strong enough to bear truth.
Though outlawed or hidden, her cult persists. Her name is whispered behind infirmary curtains, scratched into dungeon walls, and tattooed beneath the tongue. She is not evil—only honest. Pain is real. Pain reveals.

Pain is portrayed as a tall, faceless woman clad in corseted crimson robes, veiled in black, barefoot upon thorns. Her eyes are wrapped in bloodstained bandages. In one hand she carries a cat-o’-nine-tails braided from sinew and silver; in the other, a mirror with no reflection.
Her faithful wear masks during rituals: smooth porcelain, broken ceramics, or carved from their own likeness—because the first lie Pain unmasks is the self.

1. The Veil of First Flesh (20 gp)
Appearance: A simple crimson sash worn across the eyes or face.
Function: Required for 1st-level spells.

2. The Mask of the Lash (75 gp)
Appearance: A porcelain or iron mask, molded to resemble the wearer—or deliberately broken.
Function: Required for 2nd-level spells or higher.
Special Effect: While masked, gain +1 save vs charm or illusion effects.

3. The Vestment of Thorns (300 gp)
Appearance: A crimson-and-black robe lined subtly with tiny barbs along the inside hem.
Function: Required for 3rd-level spells or higher.
Optional Effect: If worn during a ritual fast or ordeal, regain 1 spell slot after inflicting or receiving 5 HP of damage (once per long rest).
