The Cavnos Islands are a pair of uninhabited islands in the Sea of Kings, just north of New Cadsoline. The islands are territory of the Empire's Province of the Sunfall Isles. Both of the islands, which are named Big Cavnos and Little Cavnos, are both rocky and somewhat mountainous in their terrain. They get their name from the abundance of caves on the island.

The multiple caverns of Cavnos form an extensive labyrinthine network, with some believing that it could extend far enough underground to reach the Underlands. The caves can be treacherous, however, with many passages being dangerously narrow and winding. Even more experienced explorers have entered the caves and faced the consequences, either getting stuck in a tight crevice and dying or getting lost and disappearing in the maze of passages under the islands. Despite the dangers, daring spelunkers still travel to the islands to explore the abundance of ruins beneath the surface.

While few are easy to reach, the ruins of the Cavnos Islands are mostly simple houses carved out from the stone walls of the caves. These houses predate most of recorded history, so it is believed that they were made in the aftermath of or prior to the events of the Cataclysm. A select few ruins have also been discovered to be much more complex than what was previously believed, with hidden mechanisms that open up to reveal secret passageways. These ruins, however, sometimes have a blend of ancient and more recent constructions, which suggests that someone could have modified them for current use.

Most recently, the Cavnos Islands were home to a major criminal investigation after the Arcane Asobrian Adventurers discovered a secret torture chamber hidden within a secret dungeon. Through the investigations of both the adventuring party and the Empire's law enforcement, it was found that the dungeon was used by Sir Nero von Strahd to kidnap and extort information from a Sarti Garris. Sarti had gone missing some time prior and was the sister of Triple A's own Cayne Garrison, and she sadly died of her injuries shortly after her discovery.
