In Curse Of Strahd, the vampire lord invited Anastrasya to dine with Heralds of Dawn alongside his other current spouses, Escher Chernykh, Ludmilla Villisevic, and Volenta Popofsky. The spouses were all offended to various degrees when their husband casually extended a proposal to Ajuni Elajoral, seething at her from across the table, but continued without incident after the Unknown politely declined. Anastrasya indulged drunkenly on blood and flirted with Mormeros Fayan before departing to her chambers at the dinner's conclusion.
There, she continued to make Potions of Healing from the bodies of dead Barovians, until her cats alerted her that intruders had snuck into her quarters. She was lead to chase them outside, not realizing that Ezmerelda D'Avenir had turned them invisible, and continued to the top of the drum tower where she was followed by Mormeros. Anastrasya attempted to seduce the tiefling, who seemingly accepted, but rebuffed her with fire at the last moment. She was blasted away by Ajuni Elajoral and attempted to climb down the tower to inform Strahd, but Mormeros made a pact with the Dark Power Ten-Zras and magically commanded her to stay.
Under the influence of the Dark Power, Anastrasya returned to her quarters and found that her cauldron had been emptied. After a day, she was able to regain her senses, and informed her husband what had happened. In tandem with Rahadin's report, Strahd found his guests both amusing and rage-inducing.
Anastrasya was instructed to monitor the Vistani Camp alongside her fellow spouse Ludmilla when Strahd learned of divisions in the camp under Zandros's rule. There, she fought the adventurers again: Ludmilla charmed Izek Strazni and Ghalwin of the Grove, and Anastrasya expected a total victory when the invisible Ezmerelda was found. However, in a burst of lightning, Ezmerelda destroyed Ludmilla, removing her vampiric charm over the warriors. Zandros fled the camp, leaving Anastrasya to scream in futile rage before dying at the points of multiple swords.
